This article assists users when completing the event.

Event information

The Manifestation Determination event is created, along with the Notification of Meeting-Manifestation Determination, when the Disciplinary Documentation event is locked and Page 1 of the Discipline Documentation form indicates the current suspension is, or may be, greater than 10 consecutive days, the student has been suspended for more than 10 days this school year and a pattern has been created, or the student has been suspended for an infraction involving drugs/weapons/serious bodily injury.

A Manifestation Determination meeting must be held by the eleventh day of suspension.  The purpose of the meeting is to determine if the student’s behavior that led to the suspension is or is not related to the disability and/or a failure to implement the student’s IEP.

This event includes Sections B, C, D, and Section E of the Discipline Documentation form.  Page 1 and Section A are in the Disciplinary Documentation Event.

A required Additional Information form is included to summarize the final outcome of the disciplinary action taken by the school/district, including the number of days the student is actually suspended out of school for this incident and the total for the year. Other outcomes that may be included, as appropriate, include IEP/Amendment scheduled or completed, reevaluation requested, etc.

NOTE: If a Manifestation Determination event is required but is not created when the Disciplinary Documentation event is locked, contact the Service Desk for assistance.

The Notification of Meeting-Manifestation Determination, including the Record of Attempts, is associated with this event and must be completed, saved and Create/Store Invite clicked prior to locking the Manifestation Determination Event.


Additional Manifestation Determination Event is Needed When One is Already Open

Phoenix allows more than one Disciplinary Documentation event to be open at the same time but only one Manifestation Determination event can be open. If a Manifestation must be left open until a Reevaluation is completed to conduct a Functional Behavioral Assessment (FBA) and the student is suspended again, the second Manifestation Determination event cannot be created until after the first one is completed and locked. At that time the subsequent Disciplinary Documentation event can be created, completed and locked. This creates the new Manifestation Determination event. Any subsequent Manifestations can be recorded on paper and transferred into the new events when they are created. If you have questions, please contact the Service Desk.
