The personnel related to an evaluation may need to be updated in Caselog when an Evaluation Chair becomes involved in the case. The Evaluation Chair in the Personnel tab defaults to the person assigned as the “Primary Staff Member” of the initial eligibility or reevaluation event in Phoenix. The administrators default to those associated with the primary staff member in Phoenix and may need to updated.
Search for student
Select the Caselog tab
Click Select for the initial eligibility or reevaluation event process
Select the Personnel tab
Click Select for the event (if more than one choose most recent), the section expands displaying the fields for the current personnel assigned
Click in the Evaluation Chair field and begin typing the name of the assigned Evaluation Chair, select the name when it appears in the drop-down
Repeat Step 6 to update any additional role. The EPS and Direct Supervisor are associated with the Evaluation Chair. The Director and the Building Supervisor are associated with the school the student attends.
To enter a Dx SLP, Yes must be selected for Consult with DX SLP in the Suspected Disability Details on the Summary tab

The student will not appear on the Evaluation Chair Queue or Administrator Queue on the home screen until the roles are updated on the Personnel tab. For a reevaluation, it must be added to Caselog AND the roles are updated on the Personnel tab.
