This article assists users when completing the Classroom Accommodations screen.

The screen is used to enter accommodations and modifications the student will use in the classroom. The selected accommodations print on Alternate Form F. 

Information

This page includes instructions for entering program/classroom accommodations. To view information about entering accommodations for state assessments (Grade-Level Assessments, End-of-Course Assessments, ACCESS for ELLs, and NAEP) click here. Information about entering accommodations for both classroom and state assessments can also be found on the Accommodations Quick Start Card

Remember to select “Documented on Alternate Form F” in the Program Modification and Accommodations section on the Services Summary Form. This will create the Alternate Form F-Decision form. Choose “Include Alternate Form F in print layout” on the Decision Form. If this step is not completed Alternate Form F will not appear in the printed IEP, even if accommodations are entered on the Classroom Accommodations Screen. If this happens and the IEP has been locked, an IEP Correction will have to be completed.

Reminder

The Classroom Accommodations Screen is not required by Phoenix. Therefore, it is possible for the IEP to be locked without entering any classroom accommodations. This could cause Alternate Form F to be blank when printed and no accommodations would be included in the IEP. If this happens and the IEP has been locked, an IEP Correction event must be completed to correct the error. Printing and proofreading the IEP before locking can prevent this.


Adding Classroom Accommodations to the Plan

  1. Open the Classroom Accommodations screen.

  2. Select a Category to view a short list for that category or All to view all accommodations.

    1. Because the details must be completed for all selected accommodations before saving, it is suggested you enter, complete, and save a few at a time.

  3. The accommodations appear in Search Results. If "All" was selected the accommodations are grouped by category.

  4. Select the classroom accommodations.

  5. Add to Plan.


Completing Classroom Accommodation Details

  1. Click Select to choose the Instructional Areas. The Instructional Areas window opens.

    1. Select at least one area. More than one can be selected.

    2. If "Other" is selected, enter the specific area in the Notes text box. 

    3. Select OK.

    4. Hover over the Instructional Areas link to view the selected areas. 

    5. To edit the areas select the link to reopen the window.

  2. Click Select to chose the Frequency. The Frequency window opens.

    1. Select one frequency.

    2. If "Other" is selected, enter the specific frequency in the Notes text box. 

    3. Select OK.

  3. If necessary, edit the Begin Date or End Date.

  4. Click the date link. The calendar opens.

    1. Use the navigation tools to move to the correct month and year.

    2. Select the date

    3. If "Other" was selected, enter the specific accommodation in the text box.

  5. When the details for all accommodations have been entered, Save.

Deleting Accommodations

Accommodations can be deleted by selecting the delete icon. The Confirmation window will open. Select Yes. The accommodation is deleted.
