The Out-of-State Transfer Student Documentation Form is used when a student transfers to a partner district from out of state.
The SSD Intake Department completes the enrollment and demographic information and the Records sections using information provided by the Partner District. Once records are received, Intake will enter the date the IEP and/or Evaluation report were received as well as the Date(s) of the IEP/Evaluation Report. Based on information entered in the Records section, A, B, C, or D will become active.
School Psychologist:
Indicate method of provision and date Procedural Safeguards and Bill of Rights were issued to parent
Reviews evaluation information and accepts or reject it
Completes the Out-of-state Transfer Form
Completes the Disabilities Screen, if needed
Area Coordinator, SSD Building Principal or Case Manager:
Reviews IEP to accept or reject it and communicates decision to School Psychologist
Completes the Transfer Interview Form, if needed
Completes the Comparable Services Form, if needed
Area Coordinator or SSD Building Principal:
Locks the event once all steps have been completed
For an out of state transfer, if the evaluation is rejected, Phoenix will generate an Initial Joint Review/Consent event that is due within 1 month + a few additional days of the enrollment date. The due date for the event was determined by SSD at the launch of the first online special education information management system.
