This article assists users when completing the form.

Form information

Reminder - When post-secondary transition will be discussed and Form C completed during an IEP meeting, Consider Post-secondary Transition must be checked as a purpose of the meeting on the Notification of Meeting. Checking that option will autofill the student's name and title (STUDENT) and make the first Agency Name field required.

Form C: Post-Secondary Transition Plan is created when either Yes or No (However, IEP team agreed to complete a Post-secondary Transition Plan) is chosen in the Post-secondary transition services section of the Special Considerations form.

  1. For the required Employment and Education/Training sections, choose a Post-secondary Goal from the menu. 

  2. In the Post-secondary Goal text box, complete the statement After graduation, this student WILL: by entering the student's post-secondary goal for that area.

  3. Choose the appropriate Responsible Agency/Person for the transition service.  More than one can be chosen.

  4. Enter the Transition Services.

  5. Complete the Independent Living section, if appropriate for this student, following the same steps as Education/Training and Employment.

  6. Indicate how the student will graduate (earning required credits or meeting IEP goals).

  7. Enter the student’s course of study beginning with the current school year.

  8. Save.

The text boxes for the student’s post-secondary goals, Transition Services and Course of Study expand as needed.  There is no character limit on expanding text boxes.

If this form has been created, completed and saved but is no longer needed, it can be reset so it is not included in the final, locked IEP. Instructions for resetting forms are found on the Resetting Forms page.

If an IEP goal supports Transition, remember to indicate this in the Goal Details panel of the Goals Screen.
