This article assists users when completing the event.

Event information

The user creates and completes the Disciplinary Documentation Event is user created and completed event each time a student receives an out-of-school suspension. Instructions for creating the Disciplinary Documentation Event can be found here.

The Due Date and Scheduled Date are user-entered and can be the same - the date the event is created or the date of the incident.

The event includes Page 1 and Section A of the Discipline Documentation form. When the Disciplinary Documentation event is opened, Page 1 is  Page 1 is the only required form.  The responses entered on Page 1 determine if Section A or a Manifestation Determination is required.

Section A is created in the Disciplinary Documentation Event event if the student has been suspended for accumulated more than 10 days of suspension this school year but no pattern has , but no pattern of behavior has been created (no Manifestation Determination is required).

Sections B, C, D, and E are in the Manifestation Determination event that is created , if required , when the Disciplinary Documentation Event is locked.

Multiple Disciplinary Documentation events can be open at the same time. It is not necessary to lock one to create another. Only However, only one Manifestation Determination event can be open at a time. If a Manifestation Determination event is open, it must be locked before another Disciplinary Documentation event requiring a Manifestation Determination is locked.


Locking the Disciplinary Documentation Event

  1. After saving all forms, return to Event Overview.

  2. Click Validation Check.

  3. If errors appear, resolve them before continuing.

  4. When the

  1. event passes the validation check, click Print to preview and proofread the form(s).

  2. When all information is correct, click the Lock button.

If the total number of days suspended this school year is not greater than 10, or

If this the current suspension is for more than 10 days, the length of the current suspension is undetermined, or the student has accumulated more than 10 days of suspension this school year, AND a pattern of behavior has been created, Phoenix will create the creates the Manifestation Determination event when the Disciplinary Documentation event is locked. The Manifestation Determination event which includes a the Notification of Meeting , a Front Page, and Discipline Documentation Sections B, C, D, and E. An Additional Information Form form is also included to summarize the final outcome of the disciplinary action taken by the school/district, including the number of days the student is actually suspended out of school. A Notice of Action is not included in the event.
