The Evaluation Chair in the Personnel tab defaults to the person assigned as the “Primary Staff Member” of the initial eligibility or reevaluation event in Phoenix. The personnel associated with a evaluation may need to be updated in Caselog when an Evaluation Chair is needed to complete the event in Phoenix.  The administrators default to those associated with the primary staff member in Phoenix and may also need to be updated. 

  1. Search for student 

  2. Select the Caselog tab

  3. Click Select for the initial eligibility or reevaluation event process 

  4. Select the Personnel tab

  5. Click Select for the most recent event, the personnel section displays current personnel assigned

  6. Click in the Evaluation Chair field to begin typing the name of the new Evaluation Chair, select the name when it appears in the drop-down

  7. Repeat Step 6 to update any additional role.  The EPS and Direct Supervisor are associated with the Evaluation Chair. The Director and the Building Supervisor are associated with the school the student attends.

  8. To enter a Dx SLP, Yes must be selected for Consult with DX SLP in the Suspected Disability Details on the Summary tab

  9. Save

The student will not appear on the Evaluation Chair Queue or Administrator Queue on the home screen until the roles are updated on the Personnel tab. For a reevaluation, it must be added to Caselog AND the roles are updated on the Personnel tab. If multiple cases are being transferred to a new Evaluation Chair for example if the original Evaluation Chair is leaving, contact your EPS who can transfer all cases automatically.
