Event information

This event is created by the system after the Initial Joint Review event indicating an evaluation is warranted and has been locked.

Evaluation Chairs should begin completing the evaluation report prior to the eligibility conference. Information can be entered into the Background and Referral Information and Evaluation and Results Interpretation pages.  

Note

NO INFORMATION SHOULD BE ENTERED INTO THE DISABILITIES SCREEN OR INTO THE ELIGIBILITY DETERMINATION FORMS PRIOR TO THE CONFERENCE!

 

Other staff conducting portions of the evaluation are required to enter their testing interpretations/summaries into Phoenix prior to the eligibility conference. 

The Notification of Meeting-Initial Eligibility, including the Record of Attempts, is associated with this event and must be completed and stored prior to locking the Initial Eligibility Event.
