This article assists users when completing the form.

Form information

Sections B, C, and D of the Discipline Documentation Form are included in the Manifestation Determination event.  They are used when the current suspension is , or may be , greater than 10 consecutive days, the student has accumulated more than 10 days of suspension during this school year, and a pattern has been created, or a Manifestation Determination is being completed when the student has been suspended for drugs/weapons/serious bodily injury.

The Manifestation Determination Eventevent is automatically created when the Discipline Documentation Event event is locked and locked, and Page 1 indicates that a manifestation determination is required.


Section B

Section B is completed for all Manifestation Determinations.

  1. Check the box for Documentation is present that within ten (10) school days... if the meeting was held within ten school days of the decision to suspend the student.

Section C

Section C is completed when the conduct was not a manifestation of the disability.  It is required based on the choice entered in Section B.

  • Check the first two boxes.

  • If an
    1. The boxes in steps 1 and 2 are checked automatically.

    2. In Step 3, enter the date of the IEP or IEP Amendment

    2. Describe the services, educational setting, and placement provided to the student during the long-term suspension.

    3. Enter the Names and Roles of the individuals making the decision.

    4. Check the box for “Parent provided Prior Written Notice for change of placement/services . . .” if the IEP team determined a change of placement/services is required

    Section D

    Section D is completed when the conduct was a manifestation of the disability.  It is required based on the choice entered in Section B.

    Check the box tip
    1. The box for Relevant IEP team members found the conduct was a manifestation of the child’s disability is checked automatically.

  • Make the appropriate choices in each of the next 3 sections.

    1. FBA section: only one choice can be made

    2. BIP section: more than one choice can be made

    3. Placement section: only one choice can be made.

      1. If this suspension is for drugs, weapons or serious bodily injury, check both boxes in that section (Continued the child's placement and Determined services that would enable the child to)

  • If an IEP or IEP Amendment is completed to change the student’s placement check the last box:  If the IEP team determines a change of placement/services is required, parent is provided with a Prior Written Notice for the proposed change of placement/services.

    1. The Prior Written Notice is completed in the IEP event (IEP or IEP Amendment), not in the Manifestation Determination event.

  • Click the Save button.

    1. In number 1, select the appropriate option regarding a functional behavioral assessment (FBA).

    2. In number 2, select the appropriate option regarding a behavior intervention plan (BIP).

    3. In number 3, select the appropriate option regarding the placement decision made for the student.

      1. If the first option is selected (no change of placement), no further selections are needed in this section.

      2. If the second option is selected (change of placement), the box in the section is checked automatically.

      3. If the third option is selected (45 school day placement for drugs, weapons, or serious bodily injury), the three boxes in this section are checked automatically. Describe the services to be provided.

    4. Enter the Names and roles of the individuals making the decision.

    5. Save.
