Student status must be 'Active' in Phoenix
Student must have an active plan in Phoenix
Must have at least one of the following active services:
Related Services – PT
Related Services – SP
Speech
Related Services – OT
Related Services – LT
Language Therapy
Consult with Social Work
Social Work Counseling
Social Work Services
At least one of the services must be assigned to a licensed, MO HealthNet enrolled service provider
Must have an active goal associated with one of the services listed aboveSection title
