Form information
The Out-of-State Transfer Student Documentation Form is used when a student transfers to a partner district from out of state. The SSD Intake Department completes the enrollment and demographic information and the Records sections using information provided by the Partner District. Once records are received, Intake will enter the date the IEP and/or Evaluation report were received as well as the Date(s) of the IEP/Evaluation Report. Based on information entered in the Records section, A, B, C, or D will become active. School Psychologist:
