This article is to help users with the Background & Referral Information form.

Form information

The Background and Referral Information form is used as part of an evaluation report and will be the front page of the Evaluation Report when printed. 

  1. Select the Background and Referral information form from the event overview screen.

  2. Enter the Attending School District.

  3. Enter the student's Age.

  4. Enter Y/N if student is VTS.

  5. Enter Y/N if student is Non-Public.

  6. Use template information to paste into expanding text box.

  7. Save.
