Form information
The purpose of this form is to ensure Alternate Form F-: Classroom Accommodations and Modifications prints as part of the IEP.
The Alternate Form F Decision : Classroom Accommodations and Modifications form is created when 'Documented on Alternate Form F' is selected on the Services Summary form under Program Modifications and Accommodations.

If Accommodations don’t print, it’s likely that Documented on Alternate Form F was not selected on the Services Summary form. If it wasn’t selected, the Alternate Form F Decision form : Classroom Accommodations and Modifications will not be created and the Accommodations, which print on Alternate Form F: Classroom Accommodations and Modifications, will not appear in the printed IEP, even if accommodations are chosen on the Classroom Accommodations Screen. If this happens and the student requires accommodations, an IEP Correction event must be completed and Documented on Form F must be marked on the Services Summary to correct the error. Printing and proofreading the IEP before locking can prevent this. |
