Form information
The purpose of this form is to ensure Alternate Form F-Classroom Accommodations and Modifications prints as part of the IEP.
The Alternate Form F Decision form is created when 'Documented on Alternate Form F' is selected on the Services Summary form under Program Modifications and Accommodations.
Select the radio button for Include Alternate Form F in print layout.
Click the Save button.

If Documented on Alternate Form F is not selected on the Services Summary form, the Alternate Form F Decision form will not be created and Alternate Form F will not appear in the printed IEP, even if accommodations are chosen on the Classroom Accommodations Screen. If this happens, and the student requires accommodations, an IEP Correction event must be completed to correct the error. Printing and proofreading the IEP before locking can prevent this. |

If this form has been created, completed and saved but is no longer needed, it can be reset so it is not included in the final, locked IEP. Instructions for resetting forms are found on the Resetting Forms page. |
