Disciplinary Documentation Section E is included in the Manifestation Determination event.  It must be completed if the suspension is for a violation involving drugs/weapons/serious bodily injury for any number of days (it does not have to be 45 days).

1. Check each of the boxes – The parent was notified of the decision, The parent was provided a copy of Procedural Safeguards, and Within 10 school days . . . a Manifestation Determination was conducted . . ..

2. Make the appropriate selection indicating whether the conduct was or was not a manifestation of the disability.
